Linkage Demonstration Project

Overview

This free health program for adults with intellectual and/or developmental disabilities (IDD) is intended to link adults with disabilities to community health care and health promotion resources based on their assessed “unmet needs.”


How It Works

The MODHC works with county developmental disability service boards and “linkage coordinators” (selected service coordinators or community health workers) facilitate assessments/screenings. This demonstration project is active in five counties including: Boone, Washington, St. Charles, Camden and Jackson counties and aims to assess, educate and then connect people with IDD to healthcare and/or health promotion resources.

How to Get Involved/Opportunities

  • Share information on county programs with other individuals with disabilities and their families to let them now that the Linkage Demonstration Project is a free health program that teaches adults with disabilities to about:
    • Ways to be healthier
    • Regular health care
    • Important screenings
    • How to set a health goal and make a plan to meet it
    • Accessing support to connect with health services and resources in the community
