Living Well in the Community

Overview

This initiative focuses on providing evidence-based intervention (EBI) to increase participation in evidence-based health promotion programs and improve overall quality of life/health among adults with ML and adults with IDD.


How It Works

The MODHC partnered with two Centers for Independent Living (CILs) – Paraquad and Delta Center to implement Living Well in the Community, a specific intervention known for significant health outcomes for people with disabilities. The intervention has been found to increase the health outcomes for people with disabilities.

One partner, Paraquad, has more than nine years’ experience implementing the Living Well in the Community program and is mentoring the Delta Center as a new implementation site.
